Usually people who've been around here for a long time get them.
Getting one involves getting an artist to help you.
Anybody around who publicly advertises themselves as an artist seems to get bogged down with requests, so I wish you luck in finding one.

There are two types of portraits: 2D and 3D.
2D is more common, 3D is less so.
You can find a 2D one in my profile, link is above my sig.
NinjaMirage's is a wonderful example of a 3D one.

Signatures are rather cool as well.
I kind of hope that the Toratsume clan will get their own unique sigs eventually as well,
